What sciatica really is

Sciatica is pain that travels along the sciatic nerve — the longest nerve in the body, running from your lower back through the hip and buttock and down each leg. When people describe sciatic nerve pain, they often mean a sharp, burning, or electric sensation that starts in the lower back or buttock and shoots down one leg. That radiating leg pain is the hallmark sign, and it can come with tingling, numbness, or weakness in the leg or foot.

It isn’t a condition on its own so much as a signal that the sciatic nerve is being irritated or compressed somewhere along its path — commonly in the lower spine. Some people feel it flare with sitting, standing, coughing, or bending; others notice it eases when they change position. Either way, it’s your body telling you something upstream needs attention.

Common causes and symptoms

Sciatica often traces back to a disc problem or tightness and misalignment in the lower spine that presses on the nerve. Long hours of sitting, heavy lifting, an old injury, or an auto accident can all set it off. The symptoms most North Shore patients bring to us include:

  • Pain that radiates from the lower back or buttock down one leg
  • Burning, shooting, or electric-shock sensations
  • Tingling, numbness, or “pins and needles” in the leg or foot
  • Weakness in the affected leg
  • Pain that worsens with sitting, standing, or certain movements
